CONSENT TO THE PROCESSING OF PERSONAL DATA FOR MARKETING PURPOSES

I hereby give my explicit consent to SWAN, a. s., with its registered office at Landererova 12, 811 09 Bratislava, Company ID No.: 35 680 202, registered in the Commercial Register of the District Court Bratislava I, Section: Sa, File No.: 2958/B (hereinafter referred to as the “Company”), to process my personal data (to the extent of the personal data provided in this form) for the purpose of preparing and sending marketing offers of products and services provided by the Company, by persons belonging to the Company’s group, or by persons acting as intermediaries, via telephone, fax, SMS, e-mail, post, or by any other appropriate means. This consent is valid for a period of one (1) year from the date of its granting. I acknowledge that I have the right to withdraw this consent at any time, but not retroactively, in the ways specified below.

I acknowledge that the Company processes my personal data in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council on the protection of natural persons with regard to the processing of personal data and on the free movement of such data (hereinafter referred to as the “Regulation”), either on its own or through intermediaries. Intermediaries process personal data on behalf of the Company to the extent and under the conditions agreed with the Company. The list of intermediaries processing personal data on behalf of the Company is published on the websites www.4ka.sk or www.swan.sk and is regularly updated.
 

By submitting this form, I also confirm that I have been informed of the following facts pursuant to Article 13 of the Regulation:

• Identification details of the Company: SWAN, a. s., with its registered office at Landererova 12, 811 09 Bratislava, Company ID No.: 35 680 202, registered in the Commercial Register of the District Court Bratislava I, Section: Sa, File No.: 2958/B; Company’s contact telephone number: 0650 123 456 in case of fixed services, 0950 950 950 in case of mobile services.

• Contact details of the person responsible for personal data protection in the Company: dpo@swan.sk, postal address: Landererova 12, 811 09 Bratislava.

• Purpose of personal data processing: sending marketing offers of products and services provided by the Company, by persons belonging to the Company’s group, or by persons acting as intermediaries.

• Transfer of personal data: The Company currently does not transfer personal data to a third country. In the case of intermediaries established in third countries, an agreement has been concluded with the relevant intermediary containing binding corporate rules (BCR) as well as the standard data protection clause adopted by the European Commission, even if the data processing currently takes place within the European Union. Should the transfer and processing of personal data outside the territory of the European Union become necessary in connection with the Company’s data processing, the Company assures you that, in the case of transfer to third countries, it will adopt appropriate safeguards to protect your transferred personal data, privacy, and fundamental rights and freedoms, in accordance with applicable legal regulations, which will be published on the Company’s website.

• Retention period of personal data: four (4) years from the date of granting or withdrawal of consent.

I have the right to:

(i)   request access to my personal data from the Company pursuant to Article 15 of the Regulation,
(ii)  request the rectification of personal data pursuant to Article 16 of the Regulation,
(iii) request the erasure of personal data pursuant to Article 17 of the Regulation,
(iv) request the restriction of processing of personal data pursuant to Article 18 of the Regulation,
(v) object to the processing of personal data pursuant to Article 21 of the Regulation, and
(vi) request the portability of personal data pursuant to Article 20 of the Regulation.

• I have the right to withdraw this consent at any time by delivering a written withdrawal by post to the Company’s registered office address as entered in the Commercial Register, or by sending an e-mail with the withdrawal of this consent to the e-mail address suhlasy@swan.sk.

• As a data subject, I have the right to lodge a complaint with a supervisory authority pursuant to Article 77 of the Regulation.

• The collection of personal data for marketing purposes is not a legal requirement but a business requirement of the Company, and I am under no obligation to provide any of my personal data to the Company for this purpose.

• Further information on the scope, methods and purposes of personal data processing, the retention period, the rights of data subjects, and other personal data protection principles is published by the Company on its website under the Privacy Policy subpage, where it also publishes information on the designation of the Data Protection Officer and their additional contact details.
